Biobank at UMC Utrecht
In a study that is subject to the WMO and that is submitted to the MREC for review under said act, bodily material from patients of UMC Utrecht may be used, and this bodily material may be stored for future research that is still unknown. In this case, this biobank component of the file will be reviewed by the MREC at the request of the Biobanks Review Committee (TCBio) based on UMC Utrecht’s Biobank Regulations.
Note: Please indicate clearly whether any extra material is being obtained for biobank purposes (combined file) or whether it only concerns material remaining from the WMO research.

For combined files you may want to consider whether a separate information sheet and informed consent form for the biobank component is desirable.

Review of the release of bodily material from the Biobank
Before we can perform any specific scientific research with your bodily material and/or data, we must first obtain approval from the independent Biobanks Review Committee.
Withdrawal from the Biobank
If you decide, after signing the consent form, that you no longer want to take part in this biobank, you can indicate this on the enclosed withdrawal form*.
Findings which are important for you.
In exceptional cases it is possible that during a study in which your medical data and/or your bodily material have been used, something is discovered that may be important for you. Such findings will be reported to the independent committee appointed by the Executive Board. This committee will weigh up all relevant interests and on this basis will decide (after consultation with the treating doctor and/or your GP, and the medical head of department of the <enter department name>) whether or not the treating doctor will inform you about this finding. If you do not want to be informed about any findings which may be important for you, you cannot take part in this biobank.
